I HEREBY EMBRACE ALL OF THE RISKS OF PARTICIPATING IN ALL ACTIVITIES ASSOCIATED WITH SCREAMHOUSE RECREATIONAL PLAY GROUND which includes any risks that may arise due to my negligence, incompetence or carelessness.

I agree to the following:

1 – I am fully aware that SCREAM HOUSE activities has potentials risks includes but not limited to; (1). Mental stress and anxiety (2). Possibility of fallen objects (3). Use of simple tools (4). Inability to escape from the rooms in allotted time (5). Probability to sustain minor injuries during the games (6). Ripped/shredded clothes. Consequently, our team will not be held responsible for any injuries sustained during any of the games in our escape rooms due to the nature of the games. However, those below age 13 or with health issues/conditions (anxiety disorder, asthma, lights/sound sensitivity, pregnant women) might choose not to withdraw from participating but at your own risk. Proceeding with the activities implies that you have no physical or mental illness that excludes you from participating in a safe manner for yourself and others. In addition, you are not under the influence of drugs or alcohol which impairs your ability to maintain safety awareness or endanger others.

2. I have carefully read this waiver and fully understood that it is a release of liability and a waiver of any right that I may have on behalf of myself or child to bring any legal action or assert a claim for inquiry or loss of any kind to SCREAM HOUSE RECREATIONAL PLAYGROUND. Should any claims be made by me, I should be held responsible for all lost wages and attorney fees incurred by SCREAM HOUSE DEFENSE TEAM in connection with the claims.

3. Conclusively, bear in mind that we have cameras placed in all rooms. However, we will not use or reproduce videos for advert purpose without your consent. The cameras are installed for the sole purpose of the game master to observe the game and give clues as needed to help your team beat the clock and game. It is also pertinent to note that there are no refunds under any conditions once you purchase your entrance ticket. Booking your appointment implies that you have read and understood the rules including the room description on the website or before then game begins.

4. PLEASE NOTE THAT THIS WAIVER IS VALID FOR THE DAY OF THE SCHEDULED EVENT.
